Study on Patterns of the Sensitivity to Various Antibiotics of N. gonorrhea

Abstract
49 strains of gonoeocci used in the study were isolated between February 1 and November 30, 1973 from the patients at the microbiology labratory, College of Medicine Hospital, Seoul National univtrsity.
5 strains of gonococci which were received from the Department of Microbiology, National Institute of Health, Republic of Korea were also included for performing sensitivity testing.
New situation has arisen in the treatment of gonorrhoea in many countries of Asia as well as in Korea owing to increased resistance of the gonococcus to antibiotics. The limits of single session therapy are drawing near in a number of areas and high and multiple doses of penicillin, or other more expensive antibiotics, are becoming reauired with attendant epidemiological and economic disadvantages. Moreover the proportions of less sensitive "circulating" strains of gonococci to penicillin have been noted recently in many world areas including the Unites States of America, Europe, Canada, etc. Both the proportion of the less sensitive strains and the range of their sensitivity has increased and good correlation has been demoastrated between treatment failure and the less sensitive organisms.
It is the purpose of the study to determine to what extent the strains of gonococci isolated recently in this country have been resistant to various antibics which are available without prescriptions; and, if possible, to provide a helpful guide for treatment of gonococcial infections, of which newcases are increasing in year and year around the world as well as in this country. Especially due to unsuitable diagnostic laboratory facilities for isolation and identification of Neisseria gonorrhoea, only a few reports on the antibiotic susceptibility tests of gonococcus have been published in the country.
The results obtained in the study might be summerized as follows :
1. 54 strains of gonococci were isolated from 7 female and 47 male patients, which divided into 6 strains from patient urines and 48 strains from urethral discharges.
The age diatribution of cases of gonorrhoea showed that the age group of greatest rate of 46.3% is 26-30 years followed closely by 21-25 years with the next high rate of 29.6%.
The rate of 7.4% was in the age brackets 16-20 years, 31-35, years and 36-40 years respectivetively, which was followed by the 41-45 and 46-50 years with rate of 1.9%. Almost half of all cases on this occassion were among persons less than 30 years of age.
2. The in vitro action of the following antibiotics was tested against the isolated gonococci : Penicillin, streptomycin, chlorampenicol, tetracyclin, kanamycin, gentamycin, ampicillin, cloxacillin and lincomycin. It was noted that all the 54. strains of gonococci tested showed high resistance to cloxacillin and streptmycin. Rates of resistance of the gone cocci to various antitiotics were observed as follows : Cloxacillin, 63%; streptomycin, 51.9%; penicillin, 20.4%; tetracycline. 18.5%: chloramphenicol, 16.7%; kanamycin and ampicillin. 11.1% each.
3. The results for demonstrating a degree of cross resistance to the antibiotics tested of the 54 strains showed that 20 strains of them gave their resistance to single drug of 5 antibiotics, 19 strains to double drugs of 10 antibiotics, 11 strains to triple drugs of 8 antibiotics, and 7 strains to tetrad drugs of 7 antibiotics.
4. 7 strains among 11 penirillin resistant strains were considered to be relatively resistant to two drugs of Penicillin and streptomycin. 18 strains among 34 cloxacillin resistant strains appeared to be relativety resistant to two drugs of streptomycin and cloxacillin. Compared with these results, it seems that the indcidences of strains of gonococci relatively resistant to antibiotics are increasing in Korea.
